POLICIES AND GUIDELINES ON PATIENT’S BILL OF RIGHTS
Bicol Sanitarium adopts the 12 Statement on Filipino Patient’s Bill of Rights as Follows:
1. The patient has the right to considerate and respectful care.
2. The patient has the right to obtain from his physician complete current information concerning his diagnosis, treatment and prognosis in terms the patient can be reasonably expected to understand.
3. The patient has the right to receive from his physician information necessary to give informed consent prior to the start of any procedure and/ or treatment where medically significant alternatives for care or treatment exist or when the patient request information and to know the name of the person responsible for the procedures and/or treatment.
4. The patient has the right to refuse treatment to the extent permitted by law and to be informed of the medical consequences of his action.
5. The patient has the right to every consideration of his privacy concerning his own medical care program.
6. The patient has the right to expect that all communications and records pertaining to his care should be treated as confidential.
7. The patient has the right to expect that within its capacity a hospital must make reasonable to the request of a patient for services.
8. The patient has the right to obtain information as to any relationship of his hospital to other healthcare institutions in so far as his care is concerned and any professional relationships among individuals, by names, who are treating him.
9. The patient has the right to advised if the hospital proposes to engage in or perform human experimentation affecting his care or treatment and has the right to refuse to participate.
10.The patient has the right to expect reasonable continuity of care.
11. The patient has the right to examine and receive an explanation of his regardless of source of payment.
12. The patient has the right to know the hospital rules and regulations apply to his conduct as a patient.
I. RIGHT TO CONSENT
1. When the patient is admitted, he/she has the right to sign for a hospital consent. Consent forms are part of a record of medical care that is maintained for every patient. All patient visits, treatments, medications, and medical procedures are documented on the chart.
2. When the patient is admitted in the hospital, a record is begun the moment he is admitted and is kept up-to-date until he is discharge
3. Some patient’s consent include:
a. Consent for hospitalization
b. Consent for operation
c. Consent for diagnostic procedures
Note: If the patient is minor children or other special cases, the said consent be signed by parents or legal representative.
II. FREEDOM OF CHOICE
1. The patient has the right to obtain from his physician complete current information concerning his diagnosis, treatment and prognosis in terms the patient can be reasonably expected to understand.
2. The patient has the right to receive from his/her physician information necessary to give informed consent prior to the start of any procedures and/ or treatments.
3. The patient has the right to refuse treatment to the extent permitted by law, and to be informed of the medical consequences.
III. RIGHTS OF INCOMPETENT PATIENT (MINORS)
1. If the patient is minor, the parents or the immediate relatives signed the consent for hospitalization in his/her behalf.
2.The hospital personnel must explain to the parents or relatives of the patient the content of the hospital consent or consent for anyprocedures or diagnostic test.
3.The physician who admits the patient must explain to the parents or relatives of the patient the medications, treatments, procedures/tests to be given/ done to patient.
4.The parent or relatives of the patient has the right to refuse medication, treatment or any procedures to be done to patient.
5.Patient has the right to be given safe environment while in the hospital.
6.Patient has the right to receive medical care.
7.Patient has the right to receive instructions coming from the paramedical and medical professionals.
8.Patients must be greeted warmly smile to avoid shame or frightened.
9.Patient has the right to receive the truth. Personnel must not tell a lie to a child patient.
10.Patient must not be left alone in the room or during the procedures.
11.Patient has the right to choose his /her companion/watcher while in the hospital.
12.Patient and parents have the right to receive health education.
13.Patient has the right to considerate and respectful care.
14.Patient has the right to receive continuity of care.
